12mo (153 x 88mm). (Bifolium following title sprung in first quire; some minor marginal browning, heavier at end.) Contemporary calf (rebacked in sheep, worn). Provenance: John Lany (ownership inscription on blank recto of imprimatur leaf, dated 1695 and his? occasional marginalia and notes on rear flyleaf); Franz Sondheimer, British chemist (bookplate). 
 
Second edition. Includes a 9-pp. catalogue of medical books printed by Samuel Smith at end. Welcome II, 113; Wing B1086; ESTC R31572.
