The 4th Missouri State Militia Cavalry was organized at St. Joseph, Missouri, where it remained until August of 1862 for fitting out. The regiment skirmished on the Little Blue River on 2 June 1862 and then were involved in the pursuit of Colonel John T. Coffee's regiment until September. Notably, the regiment was assigned to the District of Central Missouri, Dept. of Missouri in December until 25 January 1863, during which time it defended Springfield from General John S. Marmaduke's attack on 8 January. Two regiment members were killed. The regimentwas transferred to the District of the Border in August of 1863 and was involved in the pursuit of Quantrill's Raiders from 20-28 August, and operations against General Joseph Shelby from 22 September - 26 October, including actions at Tipton, Syracuse on 10 October, Boonville on 11-12 October, Merrill's Landing and Dug Ford on 12 October, and Marshall, Arrow Rock, and Blackwater on 13 October.

In January 1864, the regiment was transferred to District of
Central Missouri, and was involved in action near Warrensburg, Johnson County on 22-24 February, and in pursuit of General Price's Army, including actions at Moreau Creek Bottos of 7 October, California on 9 October, Boonville on 9-12 October, Little Blue River on 21 October, and West Port on 23 October. After Price's defeat, the 4th engaged in continuing action at Mine Creek. Mine Creek was the only full-scale battle between regular troops fought on Kansas soil, and possible the largest of all cavalry battles of the war.

Most of the regiment mustered out at St. Louis in April of 1865, after moving to Fort Scott, Kansas to recover, and then being stationed at Sedalia, Missouri from November 1864 until April 1865.
